Insurance participation

Insurance plans and contracts with providers are constantly changing. We attempt to maintain a comprehensive list of current participation, but this is subject to change without notice. It is the patient's responsibility to know and understand their insurance coverage.

We do not take Medicaid or most Marketplace insurance plans. Our Medicare panel is currently at capacity and we are not taking new Medicare patients at this time.

HMO Reminder: If a providers name or phone number appears on the card, this must be one of the LAFM providers or phone. The patient must call their insurance company to update this.
