Lake Arlington Family Medicine has been a part of the Arlington community since 2000. Our mission is to provide you with the highest quality healthcare in a friendly, Christian, family oriented environment. We care for pediatrics to geriatrics and manage a full spectrum of important medical issues including hypertension, diabetes, high cholesterol, physicals / preventive health visits, etc. We look forward to caring for your entire family's health care needs!

Meet Our Providers

  • Dennis Poquiz, MD

    Dr. Poquiz serves as the Medical Director of Lake Arlington Family Medicine. He has been in private practice in Arlington since 1999 and is board certified by the American Board of Family Medicine. As Medical Director, Dr. Poquiz serves as the supervisor of the providers at Lake Arlington Family Medicine. When not in the office, he enjoys traveling, indoor volleyball, fly fishing and spending time with his wife and family.

  • Kristina Salcedo, NP-C

    Kristina is originally from San Diego, California. She moved to Texas in 2006. She graduated December 2017 from the University of Tyler with honors. Kristina has over 22 years of previous critical care experience. She is passionate about disease prevention and improving her patient’s quality of life. In her spare time, she enjoys spending time with her husband and three daughters. She loves to travel, enjoys crocheting and gardening.

    • Janelle Rosenberger, NP-C

      Janelle received her BSN from Texas Tech University. After 6 years of hospital experience, she completed her Master of Science in Family Nurse Practitioner degree in 2019 from The University of Texas at Arlington. Janelle strives to provide patient-centered care focusing on disease prevention and wellness. Her hobbies include cheering on her Texas Tech Red Raiders, traveling, biking, and scuba diving.

  • Debra "Chris" Ray, NP-C

    Chris Ray received her Master of Science, Family Nurse Practitioner degree in 2014, and her Bachelor of Science, Nursing degree in 2005. Both degrees are from Texas Woman's University, Dallas. She has been practicing at Lake Arlington Family Medicine since 2015. Chris enjoys helping patients achieve their optimal health through performing thorough health histories, physical exams, and by establishing valuable patient rapport. In her spare time, Chris enjoys gardening, exercising, reading, and spending time with her family and friends.

    • Daniel Purifoy, NP

      Meet our newest provider! Daniel is originally from the Houston area. He graduated from Texas A&M in 2017 with a Bachelors of Science in Biology, from the University of Texas at Tyler in 2019 with a BSN, and completed his Masters of Science in Nursing with the University of Texas at Tyler in 2024. He has previous experience working as an EMT, Emergency Room RN, and Critical Care RN at JPS Hospital. He is passionate about patient-centered care and using evidence-based practice to reach his patient’s health goals. In his spare time, he enjoys working out, trying new coffee shops, and spending time with his wife and friends.
